Site License Application

Thank you for your interest in a Site License for membership on CatholicQuiz.com. Eligible applicants (Catholic schools, parish religious education programs and other youth educational organizations) are required to complete and submit this online form to begin the process.

Applications are reviewed and approved within 5 working days from submission. Once approved, applicants will be notified by email. Your Site License will be activated upon receipt of full payment by check or money order ($2 per student based upon total verified enrollment). The minimum cost for a Site License is $225. The Site License for your school, parish or organization will be organized by number of teachers using our Classroom Scorekeeper system. Each Classroom Scorekeeper tracks up to 35 students. Teachers with more than one group (individual classes or small groups) will require more than one Classroom Scorekeeper. A high school religion teacher with a 5 period schedule will require 5 separate Classroom Scorekeepers compared to a 3rd grade teacher with 27 students who will require only one.

Please complete this worksheet to identify all teachers who will need access to CatholicQuiz.com. The maximum number of scorekeepers per teacher is five. In the instance that a teacher has any one class larger than 35 students/youth, an additional classroom scorekeeper will be provided to accommodate the number beyond 35.
